Services are individualized according to the patient’s condition, goals, plan of care, and applicable hospice or palliative-care requirements.

A personalized plan

No two patients have exactly the same needs.

The care plan reflects the patient’s symptoms, diagnosis, functional status, values, preferences, caregiver situation, and goals.

As the patient’s needs change, the interdisciplinary team reviews the plan and coordinates adjustments with the patient, family, and involved healthcare professionals.
