Residents with G-tubes require careful monitoring for aspiration risk, tube dislodgement, intolerance, dehydration, infection, and skin breakdown.

Nurse Responsibilities

  • Verify feeding order, formula, rate, flushes, medication route, and hold parameters.
  • Maintain resident positioning per order/facility policy.
  • Check tube site for redness, drainage, odor, pain, leakage, swelling, or dislodgement.
  • Administer medications according to pharmacy/facility policy.
  • Flush as ordered and per policy.
  • Monitor nausea, vomiting, diarrhea, constipation, abdominal distention, coughing, respiratory change, and aspiration concern.
  • Document intake, flushes, tolerance, site condition, and interventions.
  • Notify provider for dislodgement, suspected aspiration, intolerance, blocked tube, bleeding, infection, or uncontrolled leakage.

Competency

G-tube medication administration, feeding setup, pump use, site care, documentation, and emergency response to dislodgement.
